Consists of forms containing eight questions asked as criteria for joining the Odd Fellows along with handwritten responses. Information required included the respondent's name, residence, occupation, age, membership in other orders of Odd Fellows, health, and whether or not "a Supreme Intelligent Being, the Creator and Preserver of the Universe" existed. Tradesmen are well represented in the volume.
